On Thursday July 24, 2014 at 08:05 AM PDT, we experienced two specific power incidents to multiple blade server enclosures that impacted multiple applications and/or technologies across the campus. Active power was restored to the impacted blade server enclosures to resolve the issues. The root cause of one of the issues was attributed to human error (Data Center power audit failure). The root cause for the second issue was the result of a 3rd party hardware configuration issue. The blade server enclosure power was restored for both issues by 08:37 AM PDT, and the impacted servers, databases, and applications were available by 10:30 AM PDT. Remediation and preventative action items were identified and addressed as part of the internal root cause analysis that was conducted.
